The Applicant filed an RTI request dt.8.5.08 with the PIO, G.B.Pant Hospital.\n<\/p>\n<p>      He stated that his son Mr.Sandeep Singh was admitted in G.B.Pant Hospital<br \/>\n      on 17.11.07 for operation of left C.P. Angle large cell Meduloblastoma. Three<br \/>\n      dates viz 23.11.07, 28.11.07 and 30.11.07 were given for the operation but<br \/>\n      the patient was finally operated on 4.12.07 due to some complications<br \/>\n      generated by negligence of medical supervision and carelessness of medical<br \/>\n      authorities.   However, the patient was discharged on 26.12.07.     In view of<br \/>\n      the above, he requested for information against 11 points. The PIO replied<br \/>\n      on 18.7.08 furnishing point wise information.   The applicant filed an appeal<br \/>\n      dt. 7.7.08 with the Appellate Authority stating that he has not received any<br \/>\n      information so far.   The Appellate Authority replied on 11.9.08 stating that<br \/>\n      the reply furnished by the PIO is not proper and in order. He directed the PIO<br \/>\n      to arrange to furnish a proper reply to the applicant after obtaining the<br \/>\n      complete information from all concerned, within 15 days.      He also directed<br \/>\n      that the IPOs amounting to Rs.294\/- submitted by the applicant as fee for<br \/>\n      supply of documents as asked by the PIO be returned to the applicant. The<br \/>\n      PIO vide letter dt.26.9.08 provided additional information. The applicant filed<br \/>\n      a non-compliance petition dt.25.11.08 with the Appellate Authority stating<br \/>\n      that PIO has supplied irrelevant and uncalled for information.          On not<br \/>\n      receiving any reply, he filed a second appeal dt.23.12.08 before CIC.\n<\/p>\n<p>2.    The Bench of Mrs. Annapurna Dixit, Information Commissioner, scheduled the<br \/>\n      hearing for February 19, 2009.\n<\/p>\n<p>3.    Ms. Manju Shukla, M.S. and Appellate Authority, Dr. N.R. Laskar, CMO and<br \/>\n      CPIO, Mr. J.C. Satija, A.O., Dr. Ajai Sharma, HOD, Neuro Surgery and Dr.<br \/>\n      Shrivastava, Asstt. Prof., Neuro Surgery represented the Public Authority.\n<\/p>\n<p> 4.    The Applicant was present during the hearing.\n<\/p>\n<p>Decision<\/p>\n<p>5.    After reviewing the information provided and identifying missing information,<br \/>\n      the Commission directs the CPIO to provide, with regard to point 1 of second<br \/>\n      appeal, the C. R. number, date of admission and date of operation.          With<br \/>\n      regard to points 2 &amp; 4, information is to be provided after severing the names<br \/>\n      and any other particulars by which patients can be identified, taking recourse<br \/>\n      to Section 10(1) of the RTI Act. As regards Point 3, available information is<br \/>\n      to be provided.      With regard to point 5, information against all the missing<br \/>\n      papers to be provided.      Against point 6, the CPIO is directed to allow the<br \/>\n      appellant to inspect the file of his son on a mutually convenient date and to<br \/>\n      provide certified copies of the information required.       All information to be<br \/>\n      provided within 15 days of the receipt of this Order.\n<\/p>\n<p>6.
