Please intimate the number of Officer(s)\/official(s) who proceeded on leave to visit<br \/>\n        abroad without prior intimation and prior approval of the authority competent to grant<br \/>\n        leave during the last six years.\n<\/p>\n<p>2.      What action has been taken against such officers\/ officials who proceeded on leave to<br \/>\n        visit aboard without prior intimation &amp; approval of the authority competent to grant leave<br \/>\n        during the last six years?\n<\/p>\n<p>3.      Has any of the officers\/ officials been removed from service by the Disciplinary<br \/>\n        Authority\/HOD for said reasons?\n<\/p>\n<p>4.      It so who was the DA\/HOD for imposing penalty for removal from services for the last<br \/>\n        six years.\n<\/p>\n<p>5.      Details of officers\/officials either exonerated or censured for the said cause.\n<\/p>\n<p>6.       Photocopy of leave application 30\/04\/2004 &amp; 17\/05\/2004 &amp; its diary no &amp;date on which<br \/>\n        the same were handed over to Dealing Asstt. by during my stay in PAO-15 or LNJP<br \/>\n        Hospital New Delhi.\n<\/p>\n<p>PIO&#8217;s reply<br \/>\n        The information sought is given as under:-<br \/>\nSl.No. 1to 5. No officer\/ official (other than Sh. Arun Kumar Sharma) has ever proceeded on<br \/>\nleave to visit abroad without prior intimation and prior approval of the competent authority of<br \/>\nthis department<br \/>\n Sl. No. 6. Copy of leave application dated 30.04.2004 does not exist in the record and such the<br \/>\nsame is not provided. A copy of application dated 17.05.2004 exist on the record had already<br \/>\nbeen provided.\n<\/p>\n<p>First Appellate Authority ordered.\n<\/p>\n<p>        First Appellate Authority ordered that &#8220;It is informed that your leave application dated<br \/>\n30.04.2004 was not existing in the records of PAO-15 as well as in the office and hence no<br \/>\ninformation in this regard had been provided by the PIO.
