1. What documents are needed for the issuance of consumer card by the Not mentioned

2. By which officer action is taken on the new consumer card on Not mentioned
depositing the form and in what time action is taken and by what time
is it returned back to the consumer?
3. Within what time there is a provision for supplying ration and kerosene Not mentioned
oil after the issue of consumer cards?
4. How many new consumer cards have been issued from 2008 to 2010 The cards which are stamped are
and are ration and kerosene oil being supplied to them? Why the supplied with ration and kerosene
provision for the supply of ration and kerosene oil to the new oil
consumers not being done yet?
5 How many new consumers are there whose receipts of Rs. 25/- on old Question not clear
forms have been received by the deptt. in Regional Office.Narela from
2008 to 2010? Are their cards prepared or not?
If yes then provide the total no. If no then why and also provide the no.

Order of the FAA :
PIO(North West) is directed to provide information on points no.4,5,6,7,8, 10, 11, 12,
13, 14, 15 and 16 to the Appellant within 20 days’ time.
PIO(Headquarters) is directed to collect information from Distribution Branch on point no. 3 and provide the
same to the Appellant within 20 days’ time.
As far as Appellant’s point no. 9 is concerned, the Appellant is informed that income criteria fixed by Govt. for
becoming eligible for a BPL/AAY ration card is Rs.24,200/- per annum.
Ground of the Second Appeal:
The information provided is unsatisfactory.
Appellant has sent a letter on 06/06/2011 that he has got all the information satisfactorily and does not wish to
pursue the matter.
