Hospital by Samaypur Badali Police station were medically checked up? Provide
Name, Address, time and police station of persons.
2. Provided details of all staff and officials concerned with the hospital and their
responsibility conduct rules.
3. Name, post, Room No., phone No., Address of those staffs/officials who issue
handicapped certificate.
4. Provide details of document, particular day used for making Handicapped-
Certificate. Name, address of all application that are under consideration from
31.10.2007 to 31.10.2008 and before31.10.2007.
5. Details of all staff/officials appointed (temporary and non-temporary) in this hospital.
Give details of vacant posts with date.
6. Provide details of all facilities for general patient. Let know day, date, time and name
of chairman of the department and phone number.
7. What are facilities for which treatment of the patient and since when? Give in details.
8. Which vaccines are necessary for a patient and which are available at.
9. Make available all information in Hindi with CD.
Reply of PIO:
Information had been provided to the Appellant. The PIO had asked for payment of additional
fee of Rs. 400 for 200 pages of information.
Grounds for first appeal:
In reply of PIO it was written to Police to answer the some part of information that had not been
provided till now.
