The Appellant had asked in RTI application for a Xerox copy of
the Eye Vision Medical Report on dated 05.02.2008, 26.02.2008,
26.08.2008 from the PIO.

Particulars of the information required:-

a) M.C.No. 1140/AR dt. 2&3 Dec 1998 (Physical fitness
Certificate Counterfoil)

b) M.C.No. 1140/AR dt. 2&3 Dec 1998 (Physical fitness certificate
original)

c) Xerox copy of the Distant Vision Power Report(Right and Left
eye) which was issued on the basis of M.C.No 2227/BCR dt.
27.02.2002.

The PIO replied:-

The employee had been declared medically unfit for the post of
ASM vide medical certificate No. 1140/AR/RE-6/18 LE-6/9 dt.2/3-12-1998
due to distant vision and the counter foil of the said medical is also not
traceable.

The PIO has given a copy of the medical certificate pertaining to 1998 to the
appellant. Inspite of diligent searches the other records have not been found.
The PIO states the appellant has also been given an opportunity to search the
records but they are not available.
