Sl. Information Sought Reply of the PIO
1. I had applied for a Passport on 11.01.2011 vide File no Enclosed.
DLI A 002002. Please tell me the daily progress made on
my application so far i.e. when did my application reach
which Officer, for how long did it stay with that Officer &
what did he do during that period?
2. Please give the names & designations of the officials who There are so many passport issuing
were supposed to take action on my application & who authorities. The officers who have taken
have not done so? action in your file is mentioned in (1) above
as progress sheet.
3. Online status of my application states-“Less than 1 year Since incomplete reports have been
stay. Please send 3 sets of PP Forms for verification at received then as per the police records your

4. Please state why is it that when 10th Class certificate( for Please provide 3 PP Forms so that we have
Date of Birth) Voter’s ID Card (Address proof), PAN your address and contains your present
Card (ECNR), M.Sc Certificate (for ECNR),Marriage address rather than address given in the

Certificate, Marriage Affidavit ( Annexure D) of self & application form. Otherwise send a copy of

Incomplete and unsatisfactory information provided by the PIO despite the order passed by the FAA.

The following were present
Appellant : Dr. Sunil Wadhwa representing Mrs. Harpreet Wadhwa;
Respondent : Mr. P. Roychaudhuri, Advocate on behalf of Mr. Anurag Bhaushan, Public Information
Officer & Passport Officer;
The Appellant had sought information about her passport. The PIO has stated that PP Forms were
required since her stay had been for less then a year in the last stay of residence. The appellant states that
she has sent these forms on three occasions. The respondent states that these have not been received. The
respondent has assured the Commission that if the Appellant comes with three set of PP Forms alongwith
a covering letter tomorrow (i.e. 17/11/2011 at 12.00noon) the deficiency would be made up.
Decision:
The Appeal is disposed.
This decision is announced in open chamber.
Notice of this decision be given free of cost to the parties.
Any information in compliance with this Order will be provided free of cost as per Section 7(6) of RTI Act.
